    Upon Completion of Clinical

    Clinical instructors must collect two forms from the students upon completion of clinical:

    1. National VA Learners’ Perceptions Survey  
    2. Atlanta VAMC Student evaluation form

    Both forms must be submitted to Jeanne L. Overby within 3 days of completion of clinical.  You can e-mail jeanne.overby@va.gov or fax 404-728-7733.  Please email Jeanne when you fax the documents, the fax machine is not in her office.

    Badge Pickup

Contact your Personal Health Care Provider or you can have the test done at the KSU Clinic, the cost is $17.00 (may be less depending on insurance) / call 470-578-6644 to schedule an appointment (The updated PPD must be submitted with your clinical paperwork).
        • Positive PPD: Chest X-ray or radiology report within the last 12 months and documentation of taken prophylactic medication
        • Quantiferon: Must be negative result
      • Proof of tetanus immunization
      • Proof of Varicella or titer value report
      • Proof of two MMR immunization or titer value report
      • Proof of three Hepatitis B immunization or titer value report
      • Current Flu documentation

    Please remember that any new instructors to Children's must have all of their orientation requirements completed prior to starting on the unit with their students.

    New pediatric faculty please contact Linda Sutton, KSU clinical faculty coordinator. Her contact information is lsutton7@kennesaw.edu, 470-578-6671. Do not submit materials or contact CHOA until you speak with Linda Sutton. She will help you with the process.

    1. If you have been assigned to CHOA you will complete a clinical instructor request form. This form can be accessed via a link sent to you from Linda Sutton. Once this link is completed you will receive an e-mail from Symplr. Credentialing for CHOA will cost $49. Send your receipt to Lisa Longeiret llongeir@kennesaw.edu for reimbursement.
    2. If you do not receive Symplr’s e-mail please contact Lisa Longeiret  llongeir@kennesaw.edu
    3. Please read Symplr’s instructions carefully, detailed information is listed for required uploads and documentation.

    5. CHOA does their own background check through Symplr, you can print your drug screen from Advantage Students and upload the results to satisfy the drug test requirement.
    6. CHOA encourages you to download the Symplr Mobile App (after connecting to their site you can upload required documents and it will provide you with real time compliance status).
    7. Symplr will show green light status when you have completed each portion of credentialing.
    8. Once this is complete, you will receive an email from CHOA to attend a Patient Care Provider Orientation (PCPO). Any clinical instructors who are not current CHOA employees are required to attend.
    9. For instructors without previous EPIC experience, an 8-hour class will be required prior to the start of clinical. For instructors with previous EPIC experience a "test out" option will be offered.
    10. All clinical staff are required to complete an annual OEA CBT that will be assigned through Workday.
    11. All instructors are required to complete an 8-hour shift on their assigned unit.  This can only be scheduled once clearance is given from CHOA. This orientation is be completed no later than 2 weeks prior to start of clinical.

    Scottish Rite

    Clinical Instructors/Faculty/Clinical Groups/Practicum Students can park in the main deck. The badge will grant access to parking deck through Employee Entrance/Exit.

    Egleston

    Clinical Instructors/Faculty/Clinical Groups/Practicum Students cannot park in the main decks (T1 & T2) during peaks hours. Designated off-site parking for students has been identified at the CHOA Administrative Park located on Tullie Circle. Shuttles operate from this location on Monday through Friday, 5:00 AM to 9:00 PM.

    In order to park at the Administrative Center (spaces surrounding the building formerly known as Piccadilly), staff must contact the Egleston Parking Department at 404-785-6696. Advanced notice is required to Julie Sandock, Julie.Sandock@choa.org, in the Security office. Students must complete a car registration form and obtain a decal prior to parking at Piccadilly (Exception: During the weekend or those working an overnight shift, instructors/students may park in either of the main hospital decks. In order to do this, one must pull a ticket and have it validated at the front desk).

    Hughes Spalding

    Clinical Instructors/Faculty/Clinical Groups/Practicum Students cannot park directly at the Hughes Spalding deck. Students and faculty are to park at the Butler deck (148 Jesse Hill Jr. Drive, Atlanta 30303; just past Grady Hospital, next to McDonald's)—enter on visitor side, pull a ticket, and bring ticket with them for validation.

    • There is a required in-person orientation for this site. The date and time will be given at placement or once documents are submitted.

    Parking / Badge Information

    Emory University Hospital Midtown Only:
    Badge office hours are M-F 7-3:30 (closed for lunch from 12:30-1:00), located on the F level of the Glenn Building (adjacent to the visitor parking deck).
    Students are required to find parking in other pay lots. There is a lot at the corner of Peachtree and Pine for $5 per day if in by 9:00 a.m.

        • Positive PPD:  Negative Quantiferon Gold within 60 days prior to clinical start date (May also need a CXR if in high risk area like OB)

        • Positive Chest X-Ray:  Need evidence of INH or Rifampin treatment and negative Chest X-ray within one year of placement

    Northside Parking

    All students and faculty are to park at the Orange Parking Deck. Each student and faculty will be assigned a white parking access card to the Orange Deck. These cards would need to be recycled for Security has only given us a very small amount. All students and faculty need to turn in their cards to Student Placement Office located at Interchange Building where they pick up their badges on their last schedule shift.

    Faculty will need their cards to come in and park however they do not need their cards to leave the parking deck as long as they leave by the Hollis Cobb exit on the 3rd floor. If we do not receive the cards back we will not be able to assign new access cards to your next group. Please assist us by sharing this information with your Senior Practicum students.

    The only exceptions are those whose last shift falls on the weekend where our offices are closed. They need to drop off their parking access cards to the security office and let them know that they are students turning in their cards so that the officers will place them in the right bin for us to pick up on Monday. We would appreciate if the student would either text or email me when she had done this.

    All faculty new to Piedmont must complete general nursing orientation prior to the start of any clinical rotation.  Proficiency of nursing skills and the use of equipment must be demonstrated during orientation.  All nursing faculty working with students at Piedmont Healthcare must demonstrate proficiency of nursing skills and use of equipment annually.  Nursing Faculty with a 6 month lapse of clinical rotations at Piedmont Healthcare, must complete general nursing orientation prior to the start of any clinical rotation.  For faculty who have been working with students at Piedmont without a lapse, proficiency of nursing skills and use of equipment must be demonstrated prior the start of the Fall 2009 clinical rotations and must be completed every fall thereafter.  All new faculty must complete orientation paperwork found on the student orientation web site in lieu of attending new employee orientation.  All faculty must complete orientation paperwork annually.

    Orientation

    Must have flu vaccine for current flu season. Faculty and Students will not be able to participate in a clinical without the flu shot. NO EXCEPTIONS. See WellStar Heath Requirements.

    • Fall semester flu documentation must be submitted by October 1st
    • Spring semester flu documentation must be submitted with your clinical paperwork. 

    Unit Contacts

    Faculty who will be on site with a cohort of students such as undergraduate nursing will spend 4 hours on the unit if they have not proctored students on that unit previously.  Faculty should make a contact  with the unit a few weeks before the beginning of the clinical rotation, introduce themselves and arrange for a tour and unit orientation before the first day of clinicals with the students.  Submit Faculty Unit Observation to the nursing department (Click Here).

    Please note some managers have provided their personal cell phone numbers. Only use these numbers if absolutely necessary and do not distribute them to anyone else. 

    Professional Appearance

    1. Check with your unit to see it they have any specific dress requirements
    2. WellStar badges must be worn at all times while on campus
    3. Artificial nails cannot be worn by employees providing patient care and/or the processing and management of patient care equipment, medical or food. Artificial nails include any substance or device applied to natural nails to augment or enhance nails. They include, but are not limited to, bonding, overlays, inlays, wraps, tips and/or attached decoration
    4. WellStar nurses are identified by wearing navy blue. Students will wear their school uniforms

    Badges

    Faculty and Students should pick up their badges from Kennestone Security before they arrive for their first clinical rotation. Arrive no more than three days prior to your first rotation date. If there is a problem, call KSU Nursing. 

     

    Please bring a copy of your proof of flu vaccination (students too) when you pick up your badge. WellStar will not hand out badges without proof of flu vaccination at the time of badge pickup.

    Site and Unit Specific Information

    In addition to the WellStar prerequisite requirements, some units provide specific instructions to facilitate a positive student experience for all

    Parking:

    Kennestone – park in the employee parking garage off of North Ave
    Douglas – park in the employee lot or around the A-D buildings located behind the hospital (no parking in front of the hospital or the emergency room parking area)
    Windy Hill – park in employee parking in the rear of the hospital (please do not park in the front of the hospital)
    Cobb – park behind the hospital north of the Children’s Nest off Hospital Drive West
    Atlanta Medical Sites – report first to the Security office for badge in the 315 building, 1st floor; then go to parking office 340 building, 3rd floor.
